1. Animal companion/guardian
Does your character have an animal guide, like Luna and Artemis and Diana? If so, what kind of animal is it, what is his or her name, and what role exactly does it play in your character's life? If your character has an animal guide, there is a section on the form to describe it to the rest of us! And remember! Be unique! Don't just go with a cat or dog. Try birds, turtles, lizards, or even a horse. It really depends on your character. A senshi located in a city couldn't very well have a horse, but a bird or other small animal would make sense.

2. Tattoos and piercings. Describe them and where they're located.
3. Scars and other dermatological issues, like patches of discolored skin or odd birthmarks. For example, hotaru in the manga has some really strange looking scaring.
